Delivering for Health states the following: 'Our vision for the NHS is to re-apply its founding principles with vigour to meet the needs of the people of Scotland. Delivering for Health means a fundamental shift in how we work, tackling the causes of ill-health and providing care which is quicker, more personal and closer to home.
Some of its key actions are around enabling people with long term conditions to live healthy lives:
- increasing support for self care
- identifying those people at greatest risk of hospital admission and providing them with earlier care to prevent deterioration of health and reduce emergency admissions
It talks about setting up a Scottish Long Term Conditions Alliance in 2006 to support improvements in self management.
For Older People in 2006:
The development of an action plan for the care of older people to ensure the following outcomes:
a great integration of health and social care provision
fit for further technology to be used to support and monitor the care of older people at home
enhanced community based rehabilitation
It makes reference to robotic dispensing systems for medication regimes
In addition it makes reference to the establishment of a Scottish Centre for Telehealth
