The transformation of services is key to enabling those who wish to remain in their own home for as long as possible while living with their long term condition, social care need, or with age related independence and security needs.

The key issues affecting the transformation of services, and to enable millions more people to take advantage of these much needed services, will be covered over the course of this leading conference, including:
  • How do health, social care and housing have to change to make transformation a reality?
  • Patient care at a distance - what do clinicians really think?
  • How will the UK economy benefit from a focus on carers?
  • What do individuals really want from technology enabled services?
  • How do you commission telehealth and telecare?
  • Health and social care partnerships - can they really work?
  • How can the UK continue to be a global leader in telecare and telehealth?

Delegate places are limited, and only a handful of exhibition stands are still available.

If you are a provider of care, a clinician, or a supplier of health and/or social care services or technologies, these issues will affect you and your patients/service users. Book your place now to gain the very latest information that could have an impact on you, your organisation and the individuals who rely on you.
