A very present danger, the NHS Alliance report published in March 2007, painted a very disturbing picture. The national survey of 650 GP practices showed that not only is the clinical care of NHS patients following their discharge from hospital being put at risk, but their safety is also being threatened. GPs are plagued by delays in the provision of essential information and by inadequate detail when it finally arrives. And funds that are already in short supply are wasted on avoidable re-admissions.
This event aims to cover all aspects of electronic patient-discharge summaries and focus on integration between community and hospital services. In particular, it will give an up-to-date overview of current methods, examine some of the local and national difficulties that need to be overcome and consider ways in which major improvements can be made.
A series of keynote presenters, case histories and practical sessions will
- discuss and review clinical pathways and information flows to provide truly joined-up, patient-centred care.
- examine realistic solutions from healthcare providers that are successfully using ICT to integrate and produce effective and timely electronic discharge summaries;
The accompanying exhibition will offer delegates the opportunity to meet key solution providers, who will be showcasing their products and services and promoting the benefits they offer.
