How does it work?
The Telecare Vision

- Range of telecare services from home security to telemedicine e.g. vital signs, monitoring for long term conditions.
Telecare Equipment
- Must deliver an integrated and sustainable service,
- Meet the needs of the Service User,
- Be flexible, easily understood and maintained.
R2R (Referral to Response) Model
- Referrals for telecare are received by the Telecare Service Provider
- A needs and risk assessment is carried out at the home of the service user
- The technology and support response mechanisms are explained to the service user
- The technology is installed and tested
- The Telecare Response Centre receives and responds to calls from the telecare sensors installed in the home of the service user
- Information on the service user, the assessments, the technology installed, calls made and responses provided to the service user are held within the Telecare Response Centre database and used for future assessments of the service user and future planning and development of services in local communities.
